Once upon a time Starscream and Knock Out had the brilliant idea of mixing dark Energon with Synthethic energon to create a super soldier. To no one surprise, the plan backfired and they ended with a really creepy, really strong, undead mech that could put alien to shame in a contest about the creepiest tongue ever.
As the zombiapocalypse happened and things went out of hand, havoc broke loose on the nemesis and the number of zombie vehicons increased. Sneaky little things, one of those managed to scar Knock Out’s precious finish and leave more than a simple scratch. He got away alive, in fact he didn’t even notice the scratch or its effects at first.
Like it happened to Airachnid, the zombie infection spread to him as well but he could maintain most of his mental capacities intact. At least until he started to get hungry, which didn’t take long. Soundwave was the first one to notice, not as much because the mech notices everything as it was for the fact that he was next to Knock Out at the time. Knock Out tried to attack him and Soundwave, being the efficient bastard he is, used the same bridge trick he pulled on Airachnid to send Knock Out to the still destroyed Cybertron.
Altered Personality: Personality wise he’s not all that different: cocky, arrogant and sarcastic. However, he would have a hard time dealing with the fact that he now possesses a worrisome thirst for energon that basically makes him a cybertronian vampire.
Knock Out’s going to try to keep it controlled in public and not raise suspicions, seeing what happened when he tried to attack Soundwave, but get more and more thirsty until he starts attacking people. He will most likely go for the acolytes first (who’d care about those?) to try and sooth his needs until they get out of hand again.

Altered Appearance: Nothing to notice at first glance. It’s when he gets violent and/or thirsty that two sides of his jaw split apart at the chin, revealing his new fangs and the really long mechanical tongue.
